Once you have opened the contest microsite, click on the ENTER button to access the standard contest entry form. Fill out the entry form by entering your full name, mailing address including postal code, age, phone number, e-mail address, and select from the drop-down menu, the closest city to your residence hosting a Disney Live! performance
